If the ascendant is your front door, the midheaven is the roof: what can be seen of you from the other end of town.
It is the highest point of the chart and the one with the most to say about what you end up doing.
What it actually is
The midheaven — or MC — is the degree of the zodiac that stood at the very top of the sky, crossing the meridian, in the minute you were born. It is the sky's noon, even if you were born at dawn.
And it moves fast: since the Earth turns once a day, the MC covers all 360 degrees in 24 hours, which is one degree every four minutes. That is why, along with the ascendant, it is the first thing lost when the birth time is unknown.

What it says and what it does not
The MC does not say which job you will hold. It says how you show up when you show up, and what people who do not know you closely recognise you for.
A Capricorn MC wants to be taken seriously and earns its place by persistence. A Pisces MC becomes visible through something vaguer: sensitivity, image, what soothes. A Gemini MC is known for words, and usually holds two trades at once rather than one.
Notice the gap between the MC sign and the rising sign. When they differ sharply — which is common — a person introduces themselves one way and is remembered another. That explains a fair number of misunderstandings in a life.
The other half: the imum coeli
Every axis has two ends. Facing the MC is the imum coeli, the start of the fourth house: family, a place of one's own, where you come from.
The two move together, which is why vocation and roots are more tangled than they look. Often what someone does in public is the answer — for or against — to the house they grew up in.
The MC ruler, which is where the good part is
As with the ascendant, what organises most is not the MC's sign but where the planet governing that sign landed. That planet shows the practical route recognition arrives by.

No birth time, no midheaven
Worth insisting, because it is the question that arrives most: an approximate time — it was in the afternoon — is not enough. Four minutes move one degree; half an hour can move the MC into another sign.
